As children, young people and adults journey through their lives their needs will change. Through integrated working within the council and with partner organisations we aim to provide people with access to the right service in the right place, at the right time.
Working with children, young people and their families means you'll be in multi-disciplinary teams under a single management structure set up to deliver a 'core' of services locally. Service providers are being co-located where possible, and integrated working is supported by new processes and tools, including the Common Assessment Framework, Budget Holding Lead Professional, Lead Professional and multi-agency professional development.
Whilst those working with adults will put the service user at the centre of the assessment and planning process, understanding their needs and how you can help them. The problems faced by vulnerable people in our communities are often ignored or hidden, but they are real issues that have the potential to impact upon us all. We feel that if people are properly supported, they can play a full role in our community. We are therefore committed to meeting the needs vulnerable people identify for themselves at the time they identify them. We see this as leading to a healthy, diverse community, in which everyone is included. You will be a vital link in keeping people independent and improving their well-being.
