The opportunity to improve choice and independence for vulnerable adults through individual assessment and support.
The role
Engaging with people looking to access services, you’ll conduct individual structured assessments or facilitate self-assessments to establish clear and appropriate options. Working within the enablement approach, you’ll then plan, set up and regularly review personalised, outcome-led packages of support that maximise independence and choice.
Key requirements
With an NVQ3 or equivalent qualification in a related field, you must combine experience of working in a care environment with a commitment to applying the model of supporting choice and independence for vulnerable people within the community. Excellent written and verbal communication skills are also essential.
